Start a New Chapter
A request for affiliate status as a chapter of Priscilla’s Lost and Found must be submitted in writing by the future chapter director, and should contain the following:
- Your vision and purpose for the Priscilla chapter.
- Contact information on three personal references, including the candidate’s pastor and the head of the sponsoring faith-based organization.
- Identification of at least five mentors who have committed to active involvement in the program.
- The request should not exceed two type-written, double-spaced pages and should be mailed to the address listed below:

The quarterly review of request will be conducted on the 2nd Saturday in the months of June and December. The deadline for submission will be two weeks prior. The exact dates may be determined by calling (765) 649-3733.
